Out here in rural America, your organization may have problems delivering health and social services in a fragmented delivery system. The ability to connect to those who need your services is difficult and that has a negative impact on our friends and family throughout northwest Kansas.
Join us and the Thomas County Coalition for lunch at noon, Thursday, July 8, at the Pioneer Memorial Library as we introduce Robert Deleon of Unite Us-- a national organization with offices in Garden City. Robert will talk about opportunities to join a referral network program of health and human service providers to address all determinants of local health.
With Unite Us, network providers send and receive electronic referrals, are able to collaborate with other providers, and securely share data to improve health throughout the community.
Benefits of joining the network include: easily receiving referrals of people who can utilize your services; increase visibility and collaboration with community partners; track clients' health outcomes; and access insights and data on service gaps, trends, network performances, outcomes, and more.
